| Credentials: | WA Hospital paperwork; Immunisations/vaccinations; ID check; CRC & WA WWCC; Primary Degree; Fellowship & CPD Certificate (if applicable); Indemnity; Medicare provider number; References; Current AHPRA Registration | |||
