The GPCCMP Is Coming. Here’s How to Stay Focused, Not Frustrated.

From 1 July 2025, we’ll move from the familiar structure of GPMPs and TCAs to the new General Practice Chronic Condition Management Plan (GPCCMP). On the surface, it looks like a simplification: one item number (965), reviewed three times a year (967).

But as many GPs are already finding out- “simpler” doesn’t always mean easier.

So what's really changing?

Here’s a snapshot of what matters most:

  • The old items 721 + 723 + 732  (worth ~$787.20 total/year) are now replaced by item 965 and 967 (billed 4x/year = $626.20), which is a reduction of 20.5% if billed regularly.

  • GPs must meet the new legislative requirements, which include documenting a “comprehensive assessment”, care planning, follow-up, and compliance with privacy and consent.
  • There’s no longer a linked referral to Allied Health—it’s a separate process.
  • The work must be clinically justified, documented, and team-based where possible.

On paper, it’s clear. But in Practice, many teams are struggling with questions like:

“Who’s eligible?”

“How do I write this up properly?”

“How do I fit this in without blowing up my week?”

Here’s what we’re telling our Members:

  1.  Think of this as a reset.

    Most GPs are already doing chronic care—this isn’t new work, just a new structure. So don’t start from scratch. Start from what you already do well, then layer in the new expectations.
  2. Eligibility is likely broader than you think.

    Conditions like hypertension, osteoarthritis, chronic pain, thyroid disease, and mental health disorders absolutely qualify. Many GPs have 60% or more of adult patients eligible—many just aren’t labelled yet.
  3. Don’t do it alone

    GPs who thrive in this transition will be the ones who align with their Practice Owners and admin teams. It’s about designing smarter systems that serve everyone (including you). And the delivery models must be based on a foundation of sustainability. 
  4. Compliance can be simple—if you plan for it.

Document once, document clearly, and use checklists or templated structures where you can. Overkill is unnecessary. Confidence comes from clarity.

We’re not aiming for perfection. We’re aiming for alignment, sustainability, and clarity—without overwhelm.
